Understand who may refer a member and what information supports a Health Home referral.
Referral pathways
Providers, health plans, care teams, community organizations, family members, and individuals may request a referral. Eligibility generally includes Medicaid enrollment or dual eligibility plus multiple chronic conditions, HIV/AIDS, or serious mental illness.
Submit the referral form with current contact, clinical, and service information. MP CareSolutions will review the request, confirm eligibility, and coordinate next steps with the referring organization.
